one wire goes to the ground (anywhere on the chassis) and another goes to the relay, which should be around the fuse box in the engine bay.

The T fitting is probably ok (I got the metal one). But the hose from the kit is only from the T fitting to the sensor, you need another hose...
I didn't use the washer, but I did put teflon tape on the plug. I got the t-fitting from pepboys and you still need a reducer and some hoses.
probably, but mine was on the trunk edge with a huge crease and Jeff charged me 45, so it shouldn't be that bad
